Citrulline malate is a salt of malic acid and citrulline. Malic acid present in the molecule takes part in the citric acid cycle (Krebs cycle), the importance of which is explained later. Additionally, malic acid increases energy supply to working muscles. Citrulline takes part in urea cycle, conditioning proper metabolism of proteins. The essence of the urea cycle is elimination of harmful products of nitrogen compound transformations from the organism - amino acids and purines, which cannot be stored in the organism. Citrulline is produced in mitochondria of liver cells from ornithine and then it is transported to cytosol, where it undergoes condensation with aspartate, providing argininosuccinate.

Argininosuccinate produced from citrulline is a common link for the urea cycle and citric acid cycle (Krebs cycle). Krebs cycle constitutes a basic source of energy, essential for ATP synthesis, a high-energy compound taking part in the contraction of muscle fibers. The increase of ATP influences the growth of muscle endurance.

During intensive physical effort heart and skeletal muscles use much more oxygen than in rest. In order to ensure optimal needs of these tissues for oxygen, an important role is played by the mechanisms widening blood vessels. One of such mechanisms is the secretion of nitric oxide (NO) by endothelial cells. NO causes relaxation of smooth muscles and widening of vessels and consequently the increased supply of oxygen to working muscles, better cell nourishment and improvement of circulation.

Nitric oxide is produced from arginine amino acid, which in turn is synthesized from citrulline. Citrulline is a direct substrate for the synthesis of arginine and its amount is a factor limiting the creation of this amino acid. Arginine is a relatively exogenous amino acid, which apart from increasing the synthesis of nitric oxide, also plays other functions. As a basic amino acid, arginine regulates blood pH. It also has antioxidant properties - prevents peroxidation of lipids and neutralizes superoxide anion radicals.

Nitric oxide is a gaseous transmitter, influencing the functions of the circulatory system. It decreases the tension of smooth muscular layer of blood vessels and vascular resistance and it inhibits aggregation of platelets. Nitric oxide donors are commonly used as drugs in the therapies of many cardiovascular diseases. Decreased level of NO is diagnosed i.a. in hypertension or atherosclerosis. The following factors may influence decreasing the level of NO: L-arginine deficiency and the excess of free oxygen radicals. Apart from the influence on the circulatory system, NO also takes part in regulating the immune response, the functions of endocrine system, nervous transmission or neuron development and it also improves sensitivity to insulin.
